Service Description

Book a time for a complimentary introductory consult (15 min) and I will send an intake form for you to fill out. From there, we will work together to make a plan that works for you. Pricing varies based on number of visits or services you will need. You will not be charged for the initial introduction consult (15 min), and will be provided a pricing plan specific to you that you may agree to or decline. You will not be scheduled for a plan or a payment without your consent. *ALL Weight loss plans must include zoom visits with the dietitian. I cannot provide a diet plan without follow-up. Thank you. ** Most plans will require an initial 60 min consult after the introduction visit. Custom Visits, Classes or Chronic Disease management, prices vary. Cancelation Policy

Your wellbeing is important to us. We know that at times there are things that come up that cause the need for cancellations. Cancellations made 1 hour or more before appointment time will not be charged. Cancellations made less than 1 hour before scheduled appointment time will be charged a missed appointment fee of $30 dollars.
